Hospice case managers oversee the comprehensive care of each patient. They adopt a big-picture perspective, ensuring that every aspect of the patient’s care plan is taken into account. From the moment of admission, they are deeply involved in developing and implementing the care plan.
Every two weeks, care teams meet to discuss each patient’s progress and condition. It is the case manager who schedules and coordinates these meetings, ensuring that patients receive the appropriate attention from nurses, home health aides, and physicians as needed.
Hospice case managers address the concerns of families, patients, and facilities, working with their team to ensure the communication of important information and any changes to the care plan. They assess the needs of each patient individually, as well as manage staffing and schedules to meet these needs effectively.
Beyond coordinating care, hospice case managers assist with patient education, case monitoring, discharge planning, resource management, and patient advocacy.

Social Work
While the nurses and physicians at Lion Heart Hospice attend to the patient’s medical needs, our social workers play a crucial role in supporting patients and families. They assist with major healthcare decisions, helping with advance directives, living wills, and powers of attorney, and coordinating counseling services and grief support groups.
Volunteers
While volunteers aren't formal members of the hospice interdisciplinary care team, they play a vital role in providing emotional and mental support. Many volunteers join us after experiencing hospice care personally and wish to give back.
