Lion Heart Hospice is dedicated to providing chaplain services for our patients and families who need emotional and spiritual support. We host religious events, help patients spiritually prepare for the end of life, and enhance their overall sense of comfort and well-being.

 

Our chaplain has found that the greatest service often comes from the simple act of listening. Patients and families are frequently overwhelmed with arrangements, medical bills, and caregiving responsibilities, leaving little time to express their emotions. This is where the chaplain makes a significant impact.

 

Sometimes, people just need to be heard during their most challenging times, and our patients and families are experiencing some of the most difficult moments of their lives.

 

While the chaplain cannot fix everything, their role is to sit with patients, pray with them, and be a reliable source of emotional and spiritual comfort. The chaplain is an essential part of the Lion Heart Hospice family, providing invaluable support to patients in their final weeks and days as they prepare for what lies ahead.

Our Services

Physician

Hospice physicians play a crucial role in end-of-life care. They assess the patient's condition at the hospital or their place of residence, determine the need for hospice care, and initiate the referral process to ensure the patient receives appropriate support.

Nursing

Nurses are vital to the patient's care plan, guiding them from admission through the end-of-life journey. At admission, the nurse assists the patient and their loved ones with the hospice assessment and admissions process, addressing any questions and concerns during this challenging time.

Case Management

Hospice case managers oversee the comprehensive care of patients, ensuring that every aspect of the care plan is considered.
