At Lion Heart Hospice, our physicians play a pivotal role in evaluating patients, whether in the hospital or at home, to determine their need for end-of-life care. Upon identifying this need, they make a referral to hospice, ensuring the patient receives compassionate care.

 

A physician's referral indicates that the patient has been assessed to have six months or less to live. With this referral, the patient receives an initial 90-day hospice care period covered by Medicare. If the patient lives beyond these 90 days, the physician must re-evaluate and re-certify the patient for continued hospice care every 60 days.

 

This decision is challenging for the physician and even more difficult to communicate to families. Many may perceive hospice care as a sign of giving up, but it truly means that the physician has determined that the best course is to provide the patient with dignity and comfort in their remaining days. This approach allows the patient and their family to manage their affairs and say goodbye in a meaningful way.

 

The physician sets realistic goals for the patient and educates the family on achieving them. At Lion Heart Hospice, our physicians excel in symptom and pain management, ensuring that patients live their final days comfortably and without suffering.
